Guided by our core values of Passion, Respect, and Excellence, the MDS Coordinator plays a critical role in ensuring timely, accurate, and compliant completion of all Minimum Data Set (MDS) assessments to support resident care planning, reimbursement, and regulatory compliance.

Responsibilities for MDS Coordinator:

  • Ensure timely and accurate completion of all MDS assessments in accordance with regulatory requirements and established deadlines.

  • Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.

  • Supervise MDS data entry, validation, and transmission processes to ensure accuracy and timeliness.

  • Identify, investigate, and resolve data discrepancies and validation issues.

  • Prepare and present MDS-related reports and updates to the Director of Nursing (DON) and leadership team.

  • Provide feedback to clinical teams and address operational concerns related to documentation and care planning.

  • Participate in facility surveys, audits, and regulatory reviews, providing required documentation and support.

  • Assist with audit responses and maintain ongoing compliance with Medicare and Medicaid requirements.

  • Stay current on changes to Medicare, Medicaid, and RAI/MDS regulations and guidelines.

  • Support and contribute to MDS-related quality improvement and performance initiatives.

  • Collaborate with interdisciplinary teams to ensure accurate representation of resident care needs.

Qualifications for MDS Coordinator:

  • Graduate of an accredited School of Nursing (RN, BSN, or LPN).

  • Current, active RN license required.

  • Minimum of three (3) years of clinical experience in a long-term care setting.

  • Prior MDS/RAI experience required.

  • Strong clinical assessment skills with attention to detail and accuracy.

  • Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

RN Visiting Nurse Case Manager Masonicare Home Health & Hospice - Old Lyme, CT area Day Shift / 32hrs/wk or 40hrs/wk Masonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a dedicated and accomplished Registered Nurse Case Manager local to Old Lyme, CT. The RN Visiting Nurse Case Manager ensures quality and safe delivery of nursing services to patients in the home setting. Visits are done primarily during the DAY SHIFT, but exact scheduling is flexible. Weekend Rotation will be required. Typically, this is one weekend per month or less. Masonicare's Registered Nurses work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care. They provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services. Shift/Charge Nurse – RN

Responsibilities Assist in supervising the medication program, activity in medication room and assisting the residents with self-administration of their medication. Assist residents with Activities of Daily Living and Independent with Activities of Daily Living as needed. Maintain positive relations with residents, families and physicians. Report any significant incidents and/or changes in the residents needs to the Executive Director/Resident Services Director. Assist in the completion of resident assessments and services plans. Assist in the training of assisted living staff and participates in orientation of new staff. Attend and participates in scheduled in-service programs, training programs and staff meetings.
