Registered Nurse, Treatment Coordination Center - Days

$34.45 - $51.68 / hour

OVERVIEW

Provides compassionate nursing care to patients in a clinical setting while supporting the Mercyhealth nursing mission and values. May include variable shifts, weekends, holidays, and/or on-call hours. May also float to different units based on staffing needs and clinical competency. Performs other related duties as assigned.

Schedule: 24 hours per week

Shift: Day shift, Monday–Friday; weekend call as needed; flexible hours

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Utilizes the nursing process and collaborates with interdisciplinary team members in caring for patients.

· Administers prescribed medications and treatments in accordance with approved nursing techniques, policies and procedures, standards of care, and State laws/practice act.

· Performs assessments, planning, intervention and evaluation of care.

· Documents according to standards of care.

· Develops, initiates, and executes a patient plan of care according to the individualized needs of the patient, as prescribed by Physician and/or clinic policy including patient and family instruction.

· Participates in department or unit performance improvement activities, may include shared governance.

· Utilizes clinical expertise & guidance to other units/departments in support of the patient’s longitudinal plan of care.

· Utilizes critical thinking regarding patient safety and patient experience and appropriately escalates to the appropriate chain of command.

· Participates in the professional nursing continuum process, unit practice activities and decisions and peer review process.

· Offers ideas and/or solutions to practice issues.

· Uses innovative methods and intuitive skills to develop and implement plans of care.

· Provides innovative ways to educate patient/families

· Initiates interdisciplinary collaboration to meet the patient/family educational needs.

· Actively participates in mentoring and precepting new partners to the organization.

· Positively influences the professional and organizational environments and shared governance.

· Engages in personal development and supports system-entity opportunities for continuous learning and growth including developing or revising evidence-based practice guidelines.

· IL: 20 Continuing education per year (10 of these per year are required for IL licensure) or WI: 20 Education hours

· Actively participates in a minimum of 1 departmental or unit performance improvement activities, may include shared governance.

EDUCATION AND EXPERIENCE

Graduate of an accredited nursing program

Bachelors of Science in Nursing (BSN)

OR

Associate Diploma in Nursing (ADN)/Diploma RN with a signed BSN Agreement

OR

ADN/Diploma RN with 5 years of RN experience

OR

ADN with Professional Nursing Certification

Minimum 3 years of RN experience required

CERTIFICATION AND LICENSURE

Licensed as an RN in state of practice

Approved CPR/BLS certification required within 90 days

Other certification(s) per unit requirements. For example: ACLS, PALS, PEARS, and NRP

ADDITIONAL REQUIREMENTS

Passing the Driver’s License Check and/or Credit Check (for those positions requiring).

Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check. Must be able to follow written/oral instructions.

Wound Care/Infection Control Nurse

Job Description SUMMARY: The Wound Nurse is responsible for providing primary skin care to residents under the medical direction and supervisor of the residents’ attending physician, the Director of Nursing, or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Confer with the Director of Nursing and/or other licensed nursing personnel regarding the dermatologic disorders of residents in the facility • Examine the resident and his/her records and charts, and discriminate between normal and abnormal findings, in order to recognize when to refer the resident to a physician for evaluation, supervision, or directions • Provide assessment and diagnostic services to residents. Perform an assessment evaluation using techniques including observation, inspection, and palpation • Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff • Identify manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic and viral skin infections, scaling popular diseases, and benign tumors • Assist in resident rehabilitation activities. Where require, perform range of motion, assist in ambulation, provide emotional support. Monitor the results of physical therapy activities. • Provide preventative health care services. Make recommendations to the resident concerning nutrition, prevention of common skin disorders, etc., • Make written and oral reports/recommendations to the attending physician, Medical Director, or the DON concerning the status and care of the residents • Implement and maintain established policies and procedures relative to skRN/LPNin care treatments. Interpret these to the physician, resident family members, and public as appropriate • Performs other duties as assigned QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. • LPN/RN with current unencumbered state licensure. • Wound Certification from accredited institution preferred. • Must have current CPR certification. • Must have, as a minimum, two (2) years of experience as an LPN/RN PHYSICAL DEMANDS: The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job.
