Responsibilities
Assumes primary responsibility to provide the point of access for the Palliative Care Program. Assists in the development of Palliative Care policies and procedures, treatment algorithms and outcome measures. Educates patients, families, staff and community about the Palliative Care Program. Follows up with patients to identify additional needs and outcomes.
Qualifications
Required:
- Bachelor Degree
- Registered Nurse
Desired:
- One year in program development in a clinical setting.
- Master Degree
