MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Home Health Aide (HHA)
Location: Leominster & surrounding areas
Schedule: Flexible Hours | Based on Availability 
Wage: $19/hour + Mileage Reimbursement

Key Responsibilities

  • Provide in-home support with daily living activities (ADLs) such as bathing, dressing, and grooming
  • Assist with mobility, transfers, and personal care
  • Support light housekeeping tasks and meal preparation
  • Run errands or assist with grocery shopping as needed
  • Monitor health conditions and document services provided
  • Build positive, trusting relationships with the individuals you serve
  • Ensure a safe, clean, and supportive home environment

Qualifications

  • High school diploma or equivalent; must be at least 18 years old
  • HHA or CNA certification
  • Previous caregiving experience is a plus
  • Valid driver’s license and reliable transportation required
  • Comfortable using mobile devices for documentation
  • Must pass a background check prior to hire
  • Compassionate, reliable, and strong communication skills
