Your Role as a Registered Nurse (RN) / Case Manager: Conduct in-home assessments and reassessments and develop or update individualized care plans. Provide ongoing clinical case management, follow-up visits, and timely documentation. Support and educate caregivers, respond to changes in condition, and help ensure care plans are followed. Serve as a trusted clinical liaison for clients, families, caregivers, physicians, and other healthcare professionals. Collaborate with the Director of Nursing, scheduling, and care management teams to support continuity of care. Help address client and family concerns and coordinate appropriate clinical follow-up. Partner with community referral sources and support smooth care transitions for new and existing clients. What We’re Looking For: Active, unrestricted Registered Nurse (RN) license in Massachusetts. Two or more years of nursing experience; home care, geriatrics, private-duty care, or care-management experience preferred. Strong clinical judgment, communication, organization, and documentation skills. A compassionate, dependable professional who enjoys working with older adults, families, and caregivers. Valid driver’s license, insured vehicle, and ability to travel locally to clients’ homes.
