We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Sands Point Center for Health & Rehabilitation, a premier healthcare facility located in Port Washington, New York. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission to deliver exceptional healthcare services.

Job Summary:

As a Licensed Practical Nurse, you will be responsible for providing direct patient care and supporting our interdisciplinary team in the delivery of patient-centered care. You will assess, plan, implement, and evaluate care based on age-specific components, and collaborate with other healthcare professionals to ensure quality patient outcomes.

Responsibilities:

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care

  • Assesses, plans, implements and evaluates care based on age-specific components

  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction

  • Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences

  • Administers medications and procedures per established policies and guidelines

  • Influences care outcomes by collaborating with members of the interdisciplinary team

Requirements:

  • Must have an Associate's degree in Nursing (ADN) or equivalent

  • Current LPN license in the state of New York

  • Minimum of 0 years of experience, with new grads and LPNs with limited permit welcome to apply

  • Ability to work in a fast-paced environment and prioritize tasks effectively

  • Strong communication and interpersonal skills
