Job Summary and Responsibilities

As a Patient Flow Nurse, you will optimize patient movement throughout the hospital, ensuring efficient transitions between departments, units, and levels of care. Your role minimizes wait times and maximizes bed utilization.

Every day you will assess bed availability, patient acuity, and discharge readiness, proactively managing admissions, transfers, and discharges. You'll collaborate with multidisciplinary teams, resolving logistical challenges and prioritizing patient movement based on clinical urgency and resources.

To be successful in this role, you need strong critical thinking, a comprehensive understanding of hospital operations, and exceptional communication. Your calm, swift decision-making and mediation skills in dynamic situations are essential for seamless patient progression.

  • Under the direction and supervision of a Registered Nurse, performs nursing admission assessments on VMFH patients as they are admitted through the Emergency Department, surgical or procedural setting or through the direct admit process. (50%)
  • Receives notification of pending admissions per established procedures; interviews patients to gather relevant information (e.g. weight, height, vital signs, drug allergies, presenting problems, etc) and make an initial assessment; screens, communicates and responds to patients’ needs appropriately and within scope of practice.
  • Enters patient data per established procedures in the electronic medical record.
  • Ascertains pertinent information regarding a patient’s current medication profile and communicates to receiving nurse in a timely and accurate manner.
  • Observes general and specific physical and behavioral signs and symptoms; assesses for, and effectively communicates deviations from normal conditions; recognizes urgent/emergent situations and refers them to a RN or other provider as appropriate.
  • Under the direction and supervision of a Registered Nurse, facilitates patient throughput by supporting the patient discharge process across the acute care setting. (35%)
Job Requirements

Required

  • Graduation from an accredited Practical Nursing program or equivalent recognized military training) and successful passage of the National Council Licensure Examination for Practical Nurses , upon hire and
  • Proof of confirmed acceptance into an accredited RN program, 9 - months 
  • Successful completion of the nursing program, 24 - months
  • Licensed Practical Nurse: WA, upon hire 
  • Basic Life Support - CPR, upon hire
