Registered Nurse (RN) Responsibilities

As a Registered Nurse (RN), you will provide compassionate, high-quality nursing care while promoting the health, comfort, and well-being of our residents.

  • Assess, plan, implement, and evaluate individualized resident care plans

  • Administer medications and treatments in accordance with physician orders

  • Monitor resident conditions and accurately document changes

  • Supervise and support LPNs and CNAs in delivering exceptional resident care

  • Communicate changes in resident condition with physicians and the interdisciplinary care team

  • Ensure compliance with all local, state, and federal healthcare regulations

  • Promote resident dignity, safety, and quality of life

Qualifications

  • Active, unrestricted New Jersey Registered Nurse (RN) license

Admissions RN

Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care. This position will oversee the Bergen/Essex/Union county area. The hours are Monday-Friday 11am-7pm Responsibilities Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals. Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team. Complete all required admission documentation accurately and in a timely manner. Develop and initiate individualized plans of care. Educate patients and families on hospice philosophy, services, medications, and expectations. Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers. Obtain informed consents and ensure all regulatory requirements are met. Collaborate with clinical staff to facilitate seamless transitions of care. Maintain compliance with Medicare, state, and agency regulations. Participate in on-call or weekend rotation as assigned (if applicable). Provide compassionate emotional support to patients and families during the admission process. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of practice. Minimum of one (1) year of hospice nursing experience required. Strong assessment and critical thinking skills. Excellent verbal and written communication abilities. Ability to work independently while managing multiple priorities. Valid driver's license, reliable transportation, and current auto insurance. Proficiency with electronic medical records (EMR) systems preferred.
