We are looking for triage registered nurses (RN) who are ready to serve patients and work effectively with their care team members. As a triage RN, you’ll be responsible for answering calls from hospice patients and families, assessing their needs, providing guidance, and coordinating care over the phone. You’ll schedule home visits if needed and coordinate care with the care team, facility, and the patient’s caregiver. Most importantly, you’ll use your judgment, knowledge, and experience to ensure patients receive proper and timely care.

Qualifications

  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Graduate of an accredited nursing school (BSN preferred). Multistate license as a registered nurse required.
  • Experience: Minimum three years of experience as a registered nurse in hospice required.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently, lift at least 25 lbs, and bear the weight of an average adult effectively.

In collaboration with other team members, ensures clinical supply, vaccinations, and medication inventories are readily available for clinical operations, and appropriately monitors and coordinates proper disposal according to regulatory requirements. Creates and maintains systematic monitoring process to document efficient supply and disposal procedures. In collaboration with office site Practice Managers ensures efficient workflow of clinical staff. Provides expertise with patient management systems, including EPIC workflow, functionality, and patient care procedures. Within state guidelines, performs procedures and tests. Maintains skills per the departmental competency guidelines to meet patient care and daily operational needs. This may include front desk workflows, i.e., patient registration, scheduling, phone triage, cash handling, check-in/check-out and performing scribing duties on an as needed basis.
