Job Description The Intensive Residential Support Home (IRSH) provides intensive community-based services for individuals with intellectual disabilities (ID), mental health needs, and complex behavioral health supports. Our program works to enhance the quality of life for individuals with ID, mental health needs, and complex behavioral health supports by utilizing positive behavioral supports, multi-model clinical assessments, and other therapeutic tools. **This position requires regular travel throughout the service area (mileage reimbursed) and on-call responsibilities** Shift: 3rd shift 12:00am-8:00am Monday-Friday Job Responsibilities: · Provide In-Ratio · Review MAR daily · On-Call Rotation · Ensure all daily documentation is completed / Review all Documentation / QA Spreadsheet · Daily triage · Be the lead for all staff on the floor · Ensure daily schedules are followed · Ensure meds are given accurately and signed off for – Daily MAR Review · Complete all incident reports and give them to Director. · Check and ensure all medications are refilled for clients timely · Assist in training new staff · Assist with making appointments and attending appointments if needed. · Monthly Leadership Meeting · Report any personnel issues to House Supervisor · Other duties as assigned Qualifications · Bachelor’s degree in social work, Psychology, Counseling, or another human service field with 2 years’ experience working with people who have ID and mental health and/or challenging behavior needs is acceptable and supervisory experience. OR 4 years’ experience working with people who have ID and mental health and/or challenging behavior needs is acceptable and supervisory experience. Position Summary: The direct support professional is responsible for the daily support and oversight of clients living in the Intensive Residential Support Home. The DSP provides care for individuals living with disability and/or mental health illness by providing support and assisting with daily activities in their homes. This will include working on identified goals such as meal cooking and preparation, personal cares, promoting community involvement, social skills etc. The DSP will be responsible for administering medications during the shift needed. The home operates 24 hours a day. DSP’s will be scheduled over all shifts to ensure coverage. Shift: 2nd shift, 4pm - 12am Back Half (Tues - Sat) Job Description 1. Responsible for treating all clients with dignity, respect, and value. 2. Responsible for the health, safety, protective supervision, and wellbeing of clients on your shift when working in the home. 3. Responsible to immediately report any allegations of client abuse to the Program Manager. 4. Responsible to complete all required trainings and orientations within the period specified by the Program Manager. 5. Responsible to complete all required documentation on your shift before going home, including, but not limited to, service documentation, data sheets, incident reports, and medication administration records. 6. Responsible to provide support to clients with implementation of their personal goals and objectives. 7. Responsible to provide supervision and assistance to clients with informal supports they may need help with, including but not limited to, cooking, cleaning, laundry, planning meals, shopping, and personal hygiene. 8. Responsible to assist clients with activities of daily living/personal hygiene that they absolutely are unable to do for themselves. 9. If certified to administer medications, you are responsible for the timely safe administration and documentation of medications in the MAR. 10. Responsible to maintain the confidentiality and privacy of client information. 11. Responsible to implement client behavior intervention plans when their problem behavior warrants. 12. Promptly report any unsafe conditions in the home to the Program Manager. 13. Reports changes in client’s condition or family situation to the Program Manager. 14. Accompanies clients to scheduled appointments and transports clients or running errands with or for clients. 15. Other reasonable duties as requested by the Program Manager. Will need to secure CPI/Mandt Training, Medication Manager training Benefits · Competitive Wage at $20.50/hour · Medical, Dental and Vision plan Vacation Plan · Paid Time Off Attendance Incentive Pay Eligible DSP staff may receive an additional $1.50 per hour for all hours worked within a payroll period when the following criteria are met. To qualify for the $1.50 per hour attendance incentive, the DSP must: • Work all scheduled shifts during the payroll period • Arrive on time for each scheduled shift • Not leave any scheduled shift early • Have no call-ins during the payroll period • Have no unexcused absences If any of the above conditions are not met during the payroll period, the attendance incentive will not apply for that payroll cycle. Qualifications At least a high school education or equivalent Must have a valid drivers license At least one years’ experience working with individuals with a mental illness or multi-occurring conditions Medication manager or certified medication aide preferred (will be required to be trained in medication manager) Experience supporting individuals who experience problem behaviors required.
