Hospice RN Case Manager (RN)

Location:       Waterbury Region

Status:           Mon – Fri 8 am to 5 pm, occasional backup for holiday or weekend shift

Requirements:

  • Graduate of an approved nursing education program, BSN preferred.

  • Licensure as a Registered Nurse in the State of Connecticut.

  • One (1) year experience as a professional acute care nurse

  • Home care experience preferred but not required

  • Must have current Driver’s license and reliable transportation

Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11.
