How you’ll make an impact in this role

Provide basic bedside care to long term care patients under the direction of a Registered Nurse (RN) or physician, functioning within scope of license.

  • Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
  • Administer medications and observes patients for adverse reactions to medications or treatments.
  • Perform routine laboratory tests and simple therapeutic services.
  • Educate patients/families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care. Assures resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate. 

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date, required. 

Education:

  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
    • Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

We are looking for a Licensed Practical Nurse (LPN) to care for our patients and facilitate their speedy recovery. You will also be responsible for educating them and their families on prevention and healthy habits. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be able to follow health and safety guidelines faithfully and consistently. The goal is to-promote patient’s being-by providing high quality nursing care. Responsibilities: Licensed Practical Nurse (LPN) Monitor patient’s condition and assess their needs to provide the best possible care and advice Observe and interpret patient’s symptoms and communicate them to Collaborate with physicians and nurses to devise individualized care plans for patients Perform routine procedures (bloods pressure measurements, administering injections etc.) and fill in patients’ charts Adjust and administer patient’s medication and provide treatments according to physician’s orders Inspect the facilities and act to maintain excellent hygiene and safety Provide instant medical care in emergencies Supervise and train the nursing assistants Foster a supportive and compassionate environment to care for patients and their families Expand knowledge and capabilities by attending educational workshops, conferences etc. Requirements: Licensed Practical Nurse (LPN) A minimum of 1-2 years’ experience A team player with excellent communication and interpersonal skills Responsible and compassionate Outstanding organizational and multi-tasking skills Valid nursing license in the state of Wisconsin Apply now to join our team as an LPN and help make a real difference!
