Pediatric Hematology Nurse Navigator – Outpatient

As a Pediatric Hematology Nurse Navigator at Renown Health, you'll become the trusted guide families rely on throughout their care journey. What You'll Do

As the Pediatric Hematology Nurse Navigator, you'll serve as the clinical coordinator and advocate for children receiving outpatient hematology care. Working alongside pediatric hematologists, advanced practice providers, social workers, child life specialists, pharmacists, infusion teams, and the interdisciplinary care team, you'll help ensure every patient receives exceptional, coordinated care.

You'll build meaningful, long-term relationships with patients and families while helping simplify complex care plans, remove barriers to care, and create a supportive experience throughout every stage of treatment.

In this role, you will:

• Coordinate care for pediatric patients with hematologic disorders, including inherited, acquired, and chronic blood conditions.
• Serve as the primary clinical resource and point of contact for patients and families.
• Educate patients and caregivers about diagnoses, treatment plans, medications, transfusions, procedures, and ongoing disease management.
• Collaborate with physicians and the multidisciplinary care team to develop individualized, evidence-based care plans.
• Coordinate referrals, laboratory testing, diagnostic studies, specialty consultations, infusion appointments, and follow-up care.
• Participate in discharge planning and virtual visits when appropriate.
• Advocate for patients while helping families navigate complex healthcare systems and community resources.
• Identify and address barriers to care to improve access, continuity, and patient outcomes.
• Provide compassionate support, education, and guidance throughout diagnosis, treatment, and long-term follow-up.
• Assist with the development of clinical pathways, quality improvement initiatives, and patient education resources.
• Support community outreach, education, and specialty program development.
• Partner with referring providers to ensure seamless communication and coordinated transitions of care.

Minimum Qualifications

Education

• Bachelor's degree in Nursing preferred

Experience

• Three (3) years of acute care nursing experience, preferably in pediatric hematology, pediatrics, hematology, infusion services, or a related specialty
• Experience in care coordination, case management, patient navigation, or community health preferred
• Program development or leadership experience is a plus

Licensure

• Current or eligible for State of Nevada Registered Nurse licensure

Certifications

• Current American Heart Association (AHA) Basic Life Support (BLS) certification required upon hire
• ACLS certification required for applicable position
• Specialty certification in pediatric hematology, pediatric nursing, or care coordination is encouraged and supported

Technical Skills

• Proficiency with Microsoft Office Suite, including Outlook, Teams, Word, Excel, and PowerPoint, along with the ability to navigate electronic medical records and virtual care platforms
