Role Overview

As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.

 

You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home.

 

What You’ll Do

  • Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
  • Develop and manage individualized care plans in collaboration with physicians and care teams
  • Deliver skilled nursing care, including medication management, treatments, and interventions
  • Serve as the case manager, coordinating care across therapists, aides, and physicians
  • Monitor patient progress and make clinical decisions based on changing conditions
  • Educate patients and families to support disease management and independence
  • Coordinate care transitions and discharge planning to reduce readmissions
  • Maintain timely, accurate documentation in the EMR
  • Supervise and support Home Health Aides as needed
  • Participate in on-call rotation as appropriate

 

What You Bring

  • Active RN license (or compact eligibility) + CPR certification
  • Graduate of an accredited nursing program
  • Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
  • Strong clinical judgment and ability to think independently
  • Excellent communication skills and patient-centered approach
  • Ability to manage a caseload and prioritize effectively
  • Proficiency with EMR systems
  • Valid driver’s license and reliable transportation

Overview Under the supervision of the Ambulatory Administrator or Director, Manager and Medical Director, utilizes the Nursing Process to provide care, coordination of care and patient education to a group of patients, using the medical home mode. Assists providers in the provision of quality healthcare. Assignments are performed appropriately to age and developmental level of patients being served. Responsibilities Job Requirement Expected Performance Assists the clinician in delivering quality patient care including measuring vital signs, obtaining history, initiating EMR progress note and updating allergy, medication and problem lists. Performs or assists with dressing changes, pulse oximetry, respiratory treatments, point of care testing, vision & hearing screens, and other required procedures as indicated. Administers vaccines, medications and treatments as ordered by provider and within scope of practice. If in a clinic that serves pediatric patients, ensures vaccines are stocked and in compliance with Vaccines For Children/Florida SHOTS program. Vaccines For Children responsibility applies in clinics that participate in the program. Plans and implements age appropriate care based on assessment, diagnosis, standard of care, medical treatment plan, and progress toward desired outcome. Assesses individual physical, psychosocial and developmental needs of patients. Identifies patient/family education needs for growth, development, safety, nutrition, and wellness; coordinates or delivers training and evaluates the effectiveness of the teaching plan.
