Inpatient Advanced Practice Provider (NP/PA)

At St. Francis Hospital & Heart Center, you will be part of a nationally recognized cardiac program dedicated to innovation, excellence, and compassionate care. Our comprehensive heart failure program provides APPs the opportunity to practice at the top of their license while contributing to life-saving advanced cardiac therapies.

Position Summary

St. Francis Hospital & Heart Center is seeking a highly skilled Inpatient Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Comprehensive Congestive Heart Failure Program.

The APP will provide advanced, evidence-based care to patients with acute and chronic heart failure across all stages of disease severity, including complex and advanced therapies. This role practices collaboratively with board-certified Heart Failure Cardiologists, cardiac surgeons, intensivists, nurses, and multidisciplinary specialists to deliver safe, effective, and compassionate inpatient care.

Program Overview

The Center for Advanced Cardiac Therapeutics offers comprehensive management for patients with weakened heart muscles and advanced heart failure, including expertise in:

  • Advanced medical heart failure therapy
  • LVAD and VAD management
  • ECMO support
  • Cardio-oncology
  • Cardiac amyloidosis and sarcoidosis
  • Pulmonary hypertension
  • Cardiogenic shock
  • Structural heart disease (in partnership with the Heart Valve Center)

Key Responsibilities

  • Perform comprehensive inpatient evaluations of patients with acute decompensated and chronic congestive heart failure.
  • Diagnose and manage systolic and diastolic heart failure.
  • Develop and implement individualized treatment plans.
  • Manage guideline-directed medical therapy (GDMT).
  • Provide care for patients requiring advanced therapies including LVAD, VAD, and ECMO.
  • Assist in the management of cardiogenic shock and complex cardiac conditions.
  • Monitor laboratory results, imaging, and hemodynamic data.
  • Order and interpret diagnostic tests.
  • Participate in multidisciplinary rounds.
  • Assist in advanced cardiac therapeutic interventions.
  • Participate in LVAD patient evaluation and inpatient management.
  • Support ECMO and mechanical circulatory support management (in collaboration with intensivists and cardiologists).
  • Educate patients and families regarding:
    • Disease progression
    • Medication management
    • Fluid and weight monitoring
    • Symptom recognition
    • Lifestyle modifications (including plant-based, heart-healthy diet guidance)
  • Support transitions of care and discharge planning to reduce readmissions.

Quality & Research

  • Participate in heart failure clinical trials and research initiatives.
  • Support performance improvement efforts to reduce hospital length of stay and readmission rates.
  • Maintain compliance with institutional and national heart failure guidelines.

Qualifications

Education, Licensure & Certification

  • Master’s or Doctoral degree as a Nurse Practitioner or Master’s degree as a Physician Assistant from an accredited program.
  • Current NYS NP or PA license
  • National board certification (ANCC/AANP or NCCPA)
  • DEA registration
  • ACLS and BLS certification

Experience

  • Preferred: 2+ years of cardiology, heart failure, ICU, or inpatient cardiology experience
  • Experience managing advanced heart failure therapies (LVAD, ECMO, cardiogenic shock) strongly preferred
  • Strong clinical decision-making skills in high-acuity settings
