Cleveland Clinic's Heart, Vascular and Thoracic Institute (HVTI) Department of Heart Failure for Martin Health is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing Advanced Heart Failure team.

This role provides comprehensive care for patients with advanced heart failure across both the inpatient and outpatient settings, including patients undergoing evaluation for, living with, or recovering from advanced heart failure therapies such as heart transplantation, LVADs, and other complex heart failure interventions. The APP will care for a diverse advanced heart failure population and will manage patients independently and collaboratively with Advanced Heart Failure physicians as part of a multidisciplinary team.

Minimum Qualifications:

  • Minimum of two (2) years of Advanced Heart Failure experience as a Nurse Practitioner or Physician Assistant is required.
  • • Strong foundation in cardiovascular disease management, with a demonstrated interest in advanced heart failure. A willingness to learn, seek mentorship, and develop expertise in advanced heart failure management is highly valued.
  • Ability to work collaboratively within a high-performing multidisciplinary team and manage complex patient populations.

This position offers an exceptional opportunity to care for patients across the continuum of advanced heart failure management at Cleveland Clinic Martin Health while working alongside a nationally recognized cardiovascular team dedicated to innovation, excellence, and patient-centered care.

Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.

Responsibilities: OR first assist, procedures such as dialysis catheter placement, pre and post operative care, consultations and hospital rounds, clinic.

  • Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
  • Must be able to work with collaborating physician.
  • Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board.
  • Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury which may include assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life- threatening situations, as privileged by CCF Credentialing Board.
  • May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and CCF policy.
  • Performs specialized procedures, including central line placement as well as other specific procedures as directed by the physician, following approval by CCF Credentialing Board.

  • Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.
  • Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care.
  • Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management.
  • Supports professional development of the health care team.
  • Promotes translation of evidence-based practices, guidelines, and research.
  • May conduct research studies. May assists in data collection and analysis.
  • May serve as clinical preceptor for students. May onboard new caregivers.

Education:

  • APRN 
  • Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role. 
  • Graduate of an accredited APRN program. 
  • If in neonatal care service area Master’s Degree is required upon hire. 
  • PA 
  • Graduate of an A.R.C. approved Physician Assistant Program. 
  • Master’s degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by their practicing states Licensing Board.

We are looking for an experienced and dedicated Advanced Practice Provider (APP) to join our team. This location is equipped with an onsite lab and in-house x-ray capability. Job Description AFC Urgent Care – Harper’s Point is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide comprehensive care to patients of all ages. The APP will collaborate with our team of healthcare professionals to focus on the diagnosis and treatment of acute illnesses and injuries, workers' compensation, and occupational medicine. Key Responsibilities - Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient - Interpret and analyze patient data to determine patient status, patient management and treatment - Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician - Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions - Other duties and responsibilities as assigned Qualifications - Active and Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Ohio - ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC/PC experience preferred) - Strong clinical skills - Ability to multi-task and work independently - Excellent communication and interpersonal skills - DOT and DEA Certifications Preferred
