
Remote Per Diem Family Medicine Nurse Practitioner/Physician Assistant
Job Summary
Join our growing virtual care team as a Per Diem Remote Nurse Practitioner (NP) or Physician Assistant (PA) delivering comprehensive family medicine services through telehealth. We are seeking an experienced, compassionate provider comfortable managing a wide range of acute and chronic conditions across the lifespan while delivering high-quality, patient-centered care.
This position is 100% remote to start. As our practice grows, there may be opportunities to transition to in-person clinical care and/or full-time employment.
Job Responsibilities
Provide comprehensive telehealth evaluations for family medicine patients of all ages.
Assess and document medical history, current medications, allergies, symptoms, and preventive care needs.
Diagnose and manage acute illnesses and stable chronic conditions.
Develop individualized treatment plans and provide patient education focused on prevention and wellness.
Order and interpret laboratory studies, diagnostic imaging, and other tests as clinically appropriate.
Review results and communicate findings, recommendations, and follow-up plans promptly.
Coordinate referrals to specialists or higher levels of care when medically indicated.
Accurately document all encounters in the electronic medical record in accordance with telehealth and regulatory standards.
Collaborate with physicians and clinical staff to ensure continuity of care and an exceptional patient experience.
Participate in quality improvement initiatives and maintain best practices in clinical documentation.
Qualifications
Current, unrestricted California Nurse Practitioner or Physician Assistant license.
Active American Heart Association (AHA) Basic Life Support (BLS) certification.
At least 2 years of Family Medicine, Primary Care experience strongly preferred.
Prior urgent care/telehealth experience is a plus.
Ability to independently evaluate, diagnose, and manage patients in a remote clinical setting.
Strong clinical judgment, communication skills, and attention to detail.
Ability to successfully complete all required background checks.
Bilingual English/Spanish strongly preferred.
