As a Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
About the Role:

Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote monitoring
Collaborate with facility staff to develop and implement patient care plans
Maintain accurate and timely documentation of patient visits and treatments
Build strong relationships with facility staff and patients
Participate in quality improvement initiatives to enhance patient care standards
Requirements:
Active NP or PA license in the state of practice
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
