Primary Purpose The State Traveler Provider is responsible for conducting comprehensive in-home health assessments, Annual Wellness Visits, and preventive screenings while traveling throughout Southern Michigan. Key Responsibilities In-Home Clinical Assessments Travel to members' homes and conduct comprehensive health evaluations. Perform Annual Wellness Visits and health assessments for members with chronic conditions.
