Your Role on the Team

Position Summary

The clinical nurse collaborates with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients, ensuring the coordination of care between other disciplines and support staff. Performs professional nursing duties that incorporate the psychomotor and age appropriate cognitive skills of the patient/family/significant other to assess, plan, intervene and advocate for the patient on an ongoing basis throughout the continuum of care. Will evaluate patient outcomes, effectiveness of plan of care, and readiness for discharge and revise plan or make referrals as necessary. Acts as a role model and mentor to a variety of students and nurses new to the unit. Functions as a (PCC) Patient Care Coordinator as requested by the supervisor/manager.

Required Qualifications

What It Takes to be Successful

  • PALS certification within 6 months
  • Minimum of 2 years recent ICU nursing experience or completion of 6-month UC Irvine Medical Center Clinical Nurse I, ICU orientation in this specific ICU
  • All external candidates must have a Bachelor of Science in Nursing
  • Ability to work variable shifts including evenings, nights, weekends, and holidays
  • Ability to read, write and communicate effectively in English
  • Ability to maintain flexibility and work well in a fast paced, constantly changing environment
  • Ability to maintain a work pace appropriate to the workload

Preferred Qualifications

  • Understanding of applicable regulatory requirements
  • One or more nationally-recognized specialty nursing certification(s) (e.g., CCRN, TCRN, SCRN, etc.)
  • Bilingual skills to communicate effectively with patients and families
  • Acute Care Pediatric Experience

Job Summary: Works collaboratively with an MD to coordinate and screen for the appropriateness of admissions and Continued stays. Makes recommendations to the physicians for alternate levels of care when the patient does not meet the medical necessity for Inpatient hospitalization. Interacts with the family, patient and other disciplines to coordinate a safe and acceptable discharge plan. Responsible for complying with AB 1203, Post Stabilization notification. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team, multitask and in a fast pace environment. Essential Responsibilities: Plans, develops, assesses and evaluates care provided to members. Collaborates with physicians, other members of the multidisciplinary health care team and patient/family in the development, implementation and documentation of appropriate, individualized plans of care to ensure continuity, quality and appropriate resource use. Recommends alternative levels of care and ensures compliance with federal, state and local requirements. Assesses high risk patients in need of post-hospital care planning. Develops and coordinates the implementation of a discharge plan to meet patients identified needs; communicates the plan to physicians, patient, family/caregivers, staff and appropriate community agencies. Reviews, monitors, evaluates and coordinates the patients hospital stay to assure that all appropriate and essential services are delivered timely and efficiently.
