Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.

Minimum Requirements

Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.
A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.
Experience and knowledge regarding the physical, mental, and social needs of the patients are highly desirable.
At least one year of working with a frail or elderly population
Valid California driver’s license with a good driving record.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Medical Coordinator (LVN) - Day Shift

Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is Thursday - Monday from 7:00am to 3:30pm. Workdays may vary. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems. Complete and update medication, vitals, progress notes and care plan Perform administrative tasks such as answering phones and filing paperwork. Prepare patient reports for billing.
