All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. Job Description JOB SUMMARY The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio-economic background and response to medical regimen and treatment. UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. Job Description The Pediatric Nurse Case Manager coordinates resources and services in response to individual patient’s needs and fiscal realities, performs utilization review activities and oversees coordination of complex discharge planning for assigned case load. The Nurse Case Manager is an active member of the interdisciplinary team contributing to team goal setting. The Nurse Case manager acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. The Nurse Case Manager provides age-appropriate assessments, interpretation of data, and delivery of interventions.
