Position Overview

We are seeking compassionate, experienced, and detail-oriented Registered Nurses (RNs) to join our growing team as Per Diem UAS Assessment Nurses throughout Suffolk County, NY. As a UAS Assessment RN, you will conduct comprehensive in-home assessments using the New York State Uniform Assessment System (UAS-NY) to evaluate patients' medical, functional, cognitive, and social needs. Your assessments will play a vital role in determining eligibility for home care services and ensuring patients receive the appropriate level of care.

Key Responsibilities

  • Conduct comprehensive UAS-NY assessments in patients' homes throughout Orange County.
  • Evaluate patients' physical, cognitive, psychosocial, and environmental needs.
  • Complete accurate and timely documentation within the UAS-NY system and electronic medical records.
  • Develop objective clinical assessments that support appropriate care planning and service recommendations.
  • Educate patients and family members about the assessment process and available home care services.
  • Collaborate with interdisciplinary care teams, case managers, and healthcare providers to ensure continuity of care.
  • Maintain compliance with New York State Department of Health regulations, agency policies, and documentation standards.
  • Travel throughout assigned service areas while managing a flexible schedule.

Qualifications

  • Active and unrestricted New York State Registered Nurse (RN) license.
  • Current UAS-NY Certification is strongly preferred (or willingness to obtain certification if applicable).
  • Minimum of one (1) year of nursing experience required.
  • Home Health, Managed Long-Term Care (MLTC), Case Management, or UAS Assessment experience preferred.
  • Strong clinical assessment, critical thinking, and decision-making skills.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to work independently while effectively managing appointments and documentation.
  • Reliable transportation and willingness to travel throughout Suffolk County, NY.
