Responsibilities:

• Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming

• Assist with mobility and transfer patients, as needed

• Administer medication and maintain accurate records

• Monitor and report any changes in patient condition to the Nurse, Physician, and family members

• Participate in care planning and development of individualized care plans

• Maintain a clean, safe, and comfortable environment for patients

• Collaborate with the healthcare team to ensure continuity of care

Requirements:

• High school diploma or equivalent required

• Completion of a state-approved Home Health Aide training program

• Current certification as a Home Health Aide

• Ability to work independently and as part of a team
