Health Unit Coordinator / LNA - per diem

Job Summary:

Summary

Under the general direction of the Clinical Lead/Perinatal Tech and Director, in accordance with the practice standards established by the Nursing Organization and by the specific unit/department in accordance with the rules, and regulations and/ or standards established by the State Board of Nursing, Det Norske Veritas (DNV) Accreditation and appropriate specialty organizations relevant to the patient population under care of the Licensed Nursing Assistant (LNA). 

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Practices according to the ANA guidelines of care as adopted by the Nursing Division.
  • Adheres to the Nurse Practice Act of NH.
  • Assist with admissions process within the Electronic Health Record (EHR), collaborating with the House Supervisor (HS) and Charge RN.
  • Inform the primary RN when the admission orders are entered for hand off report.
  • Assemble and label patient chart.
  • Place prior advanced directive on chart.
  • Assemble necessary forms, copy chart and book ambulance if necessary to external facilities.
  • Prepare chart and gather paperwork for discharge or transfer to other units in the hospital.
  • Schedule follow up appointments and outpatient testing.
  • Discharge patients from computer census and disassemble chart for medical records.
  • Maintain supply of chart forms, desk supplies and make packets as needed.
  • Maintain charts by thinning and adding forms as needed.
  • Send/retrieve mail and distribute.
  • Demonstrates knowledge of standard precautions, life safety codes, hospital wide codes and all safety policies.
  • Miscellaneous administrative projects for unit as directed.
  • Maintain patient census log and board.
  • Maintain call schedules and keep list daily of patients off floor for testing.
  • File forms and reports in charts.
  • Assists with recording unit/dept. statistics.
  • Prioritize orders and tasks with direct admissions known stats as priority items in collaboration with the RN.
  • Knowledge of medical terminology, symbols and abbreviations.
  • Prepare surgical and pre- procedure charts and process post procedure/post op charts.
  • Prepares unit specific packets (labor, newborn, c-section, discharge, etc.)
  • Notify staff in timely manner, ie stat orders, when blood ready for transfusion
  • Request services from ancillary and support departments for supplies or equipment.
  • Request food trays, stat meds, old records, patient’s valuables from safe etc.
  • Request patient consent for information or films from external facilities.
  • Comply with regulatory agency guidelines/rules.
  • Abide by HIPAA regulations when receiving phone calls or visitor requests for patient information.
  • Restrict access to patient information, chart, computer, and otherwise.
  • Restrict access/maintain infant safety and security of unit
  • Refer patient to patient advocate to assist in patient satisfaction.
  • Assist with patients who want advanced directives initiated or spiritual care.
  • Send and receive documents by fax.
  • Answer and disperse phone calls, in a professional and friendly manner.
  • Attends Health Unit Coordinator and department meetings
  • Enters orders and follows established computer downtime procedures.
  • Access labs, radiology reports, and other data as required.
  • Generate reports and patient education materials.
  • Notify RN if monitor alarms sound immediately
  • Duplicate documents using copy machine.
  • Transport specimens using pneumatic tube.
  • Assist with malfunctioning equipment.  Assists with monitoring of all equipment that has gone out for repair and when it has returned to the department for use.
  • Report problems with computers and office equipment to ITS.
  • Gathers pertinent patient information to complete birth certificates, and affidavit of paternity as appropriate
  • Enters data into NH vital statistics and NH Newborn Screening program sites (CCHD, Hearing Screen, etc.)
  • Assists with ensuring Newborn Metabolic Screening tests are gathered and routed in a timely manner
  • Renders nursing care to patients within an assigned unit of the hospital, in support of medical care as directed by medical staff, RNs and LPNs, and pursuant to objectives and policies of the hospital.
  • Performs various patient care activities and related services necessary in caring for the personal needs and comfort of patients.
  • Documents all care provided to the patient in the Electronic Health Record.  Such as Vital Signs, Intake, Output, Mobility, pain, and skin abnormalities.  As well as important information related to interactions with the patient and/or visitors. 
  • Reports any unusual or changes in patient condition to the primary RN and/or charge RN
  • Completes bedside shift hand-off report routinely and independently.
  • Answers call lights to determine patients’ needs in a timely manner.  Understands and demonstrates initiatives for improved response times to our patient’s needs such as NO PASS ZONE.
  • Performs skills such as delivery table set up/breakdown, handling of specimens, bladder scans, EKG’s, assists with cold therapy and other appropriate medical procedures upon completion of appropriate competencies.
  • Bathes patients daily as needed and assists with personal hygiene to increase their comfort and wellbeing.
  • Encourages and assists all patients daily with routine ambulation and mobility, unless otherwise ordered by the attending provider or instructions by the primary RN, and documents activity in the Electronic Health Record.
  • Serves and collects food trays, assists with intake for patients requiring help, and provides between–meal nourishment.
  • Collects samples for testing i.e., urine and fecal occult.
  • Assists with specialty beds and mattresses
  • Assists with skin and wound care under the direction of a nurse.
  • Expresses interest in the patient and their progress, assuring patient needs are met.
  • Uses judgment in providing information based on the situation and is sensitive to individual and organizational concerns.
  • Demonstrates effective verbal and non-verbal communication technique. 
  • Explains any nursing care given to involve the patient in their care.
  • Demonstrates willingness to adjust staffing to meet patient care needs.
  • Prepares equipment and assists other nursing staff during treatments and examination of patients.
  • Uses equipment/safety protocols and policies in providing care and transport to patients.
  • Demonstrates understanding of the fall prevention measures for all patients assessed by the RN as high fall risk.  Communicates with the RN and provides report to on coming LNA any concerns.
  • Utilizes AIDET when interacting with patients and assists with providing hourly rounding to all patients.
  • Completes all assigned annual competencies by due dates.
  • Maintains adequate patient care hours for licensure.
  • Maintains current license and certifications required at all times.
  • Avoids use of unauthorized overtime.
  • Manages ETO (Earned Time Off) per policy
  • Other duties as consistent with this role.

Job Requirements

Job Knowledge and Skills

  • Crisis Prevention Institute (CPI) training preferred or obtained prior to completion of orientation

Education and Experience

  • High School Graduate or GED preferred
  • Current New Hampshire LNA license required
  • Basic Cardiac Life Support required
  • Notary of Public required. May be obtained within 6 months of hire

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

Working Conditions/Physical Demands

Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Occasional lifting/carrying up to 25 lbs.  Some stress related to high level of responsibility for quality care.

$21.55 - $29.68
