GENERAL OVERVIEW:

This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides specialty and critical care healthcare services in physician office(s) and non- acute based settings with a focus on chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Specialty certification, advanced procedures with appropriate privileging, weekend and on-call coverage on a rotational basis, as required. E-consults and telehealth could be included in the care provided.

ESSENTIAL RESPONSIBILITIES

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation for patients in physician office(s) and non- acute based settings. 30%
  • Communicates with attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. Makes medical diagnoses and institute appropriate therapy within the applicable scope and standards of practice.15%
  • Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies appropriate for the patient's condition to determine and implement treatment plans.10%
  • Assesses learning needs of patient and family and provides education based on age, culture and willingness to learn. Educates patients and family members regarding health and illness prevention, medication and treatment instructions, and recommend community resources as needed.15%
  • Ensures appropriate understanding and timely documentation in the EMR to support accurate medical coding and billing, and record/report pertinent information to the attending physician of record.10%
  • Maintains mandatory professional continuing education, actively participate in patient safety, quality improvement, and patient engagement initiatives, and maintain professional practice consistent with organizational and advanced practice mission statements and specialty-specific privileges.10%
  • Perform other duties as assigned or required.10%

QUALIFICATIONS:

Required

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program
  • National Board Certification for Nurse Practitioners through an accredited association
  • Practicing-state licensure as a Nurse Practitioner

NURSE PRACTITIONER IV-Vascular Surgery Office

Evaluates, examines, diagnose, and treats patients in collaboration with their supervising and or substitute supervising physician according to established guidelines and protocols. Maintains and promotes the well-being of the patient providing care in both the in-patient and out-patient setting. Primarily Monday - Friday. 1. Regular, consistent, on-site, and timely attendance. 2. Ensures that the assessment of the patient and the direct and indirect patient care delivered is in accordance with appropriate licensing board, hospital policies, procedures, and protocols, and all other professional standards of care. a. Collects patient assessment data in a systematic manner, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient. b. Performs systematic physical exam and history of present illness. c. Documents assessment data, history of present illness, and physical findings on the appropriate medical record within established time frame. d. Acquires and distributes educational resources tailored to the patient's needs. d. Provides appropriate instruction and guidance regarding immunizations. 5. Communicates with supervising physician in a timely manner regarding changes in the patient's progress, status, or critical test results. g. Prepares for and develops discharge planning in conjunction with supervising physician. h. Dictates discharge summaries timely and appropriate. i. Provides counseling and instruction to patients and or families as appropriate. 3. Performs clinical responsibilities in a manner that maximizes patient care and outcomes. a. Assumes appropriate code team role during emergency situations and responds effectively. b. Facilitates timely treatment of abnormal labs or other relevant findings. c. Develops workflow patterns to facilitate patient transfers or discharges. d. Participates in outcome management and Press Ganey scores. 4. In collaboration with the physician: Interviews, evaluates and assesses patients in the clinic setting. Provides recommendations for treatment. a. Evaluates patient's current status providing recommendations for ongoing treatment. Assesses for alternate levels of care as necessary b. Adjusts medication as indicated c. Provides patient/family education regarding medication, rationale for use and possible side effects 5. Collaborates with the supervising physician and or substitute supervising physician and members of the treatment team to achieve desired outcomes and to assure continuous flow of patient care. a. Alerts the treatment team to any significant changes in the patient's status b. Within prescribed time frames reviews treatment goals and objectives with patients making modifications as appropriate c.
