Job Summary:

Responsibilities:

  • Provide medication reminders and assist with self-administration of medications

  • Offer emotional support and companionship to clients and their families

  • Maintain accurate records of client care and services provided

  • Collaborate with our interdisciplinary team to ensure seamless care and communication
