LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

 

Under the supervision of the Director, Clinic and Home Care Services, and in collaboration with the Occupational Therapist, the LPN Home Care Coordinator provides a complete and thorough assessment of the home environment and the functional capabilities of each participant in the program. The home care coordinator ensures continuity of care in the home related to ADL, IADL, and DME coordination, manages scheduling of home care services, helps to facilitate home care services during participant’s discharge from hospital or nursing home, monitors the quality of care and services being provided to the participants in the home and supervises CNA/caregiver staff. The Home Care Coordinator must be knowledgeable of the individual participant’s needs.


Qualifications

LPN Home Care Coordinator qualifications include:

  • Graduated from an accredited school of Nursing and must possess a current Practical Nurse license; in good standing, from the Commonwealth of Pennsylvania
  • Minimum of one (1) years’ experience working with a frail or elderly population. 
  • Working knowledge of the interdisciplinary model of community care management. 
  • Experience working with cognitively impaired seniors is important. 
  • Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving. 
  • Must be flexible and able to change easily.
  • Working knowledge of computers, Microsoft Word and Excel required.
