Position Summary: HealthFlex is seeking a compassionate and experienced After Hours Licensed Vocational Nurse (LVN) who strives to make a real impact for our community. By joining our team, you will be doing just that. Providing skilled services to help patients and families cope with the End of Life process.


Responsibilities:

  • Assist the Registered Nurse in carrying out the plan of care.
  • Should be willing to travel throughout the Bay Area for coverage
  • Assist Registered Nurse in performing specialized procedures (a defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician’s orders.
  • Assist with the preparation, implementation and continuing evaluation of the patient plan of care.
  • Actively participate with the IDG in collaborating to ensure the patient’s plan of care is kept current and any identified issues are addressed.
  • Help reinforce teaching to the patient/family members regarding end of life care.
  • Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other job learning.
  • Prepare clinical and progress notes as appropriate.
  • Keep the Case Manager infrared of changes noted during visits.
  • performs on-call duties (including holidays) per policy and agency need.
  • Assume responsibility of self-development by continually striving to improve his/her Nursing practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings and/or individual research and reading.

Weekend Schedule : Saturday + Sunday from 12 AM-8:30 OR Saturday +Sunday from 3:30 PM- 12:00 AM


Requirements

Qualifications:

  • Current and unrestricted LVN license
  • 1+ years of Hospice Experience required
  • 1+ years of Home Health experience a plus
  • Current AHA CPR certification
  • Driver's License
  • Able to travel to our Oakland office for weekly IDG meetings
  • The ability to communicate well, both verbally and in writing.

LVN Charge Nurse (Licensed Vocational Nurse) GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Complete all required record keeping forms/charts upon the resident’s admission, discharge, transfer, etc. File in the resident’s chart and/or forward to the appropriate department. • Receive verbal orders from attending/alternate physician and transcribe the physician’s order sheet. • Initiate resident Plans of Care on admission. • Chart TPRs, blood pressures and blood glucose as required. • Order prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Review physician’s orders and transfer/preadmission information for completeness. • Prepare/administer medications as ordered by the physician. • Answer telephone inquiries as outlined in the facility’s established policies and procedures. • Chart licensed nurses’ notes in an informative and d3scriptive manner that reflects the care provided as well as the resident’s response to the care. • Forward new diet orders, and/or diet changes to the dietary department. • Transcribe physician’s orders to resident’s charts, Medication Administration Records, Treatment Administration Records and RNA sheets to assure that they remain current. • Verify the identification of the resident before administering medication or treatment. • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician. • Assure that narcotic records are current at all times • Admit, transfer and discharge resident. SUPERVISION • Meet with unit personnel, on a regular basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Review and evaluate assigned personnel and make written evaluations to the RN Supervisor or Assistant Director of Nurses. • Coordinate Nursing service with other departments and nursing units. • Notify the attending physician, RN Supervisor and Director of Nurses when a resident is discharged without approval. Assure that appropriate documentation is made in accordance with the facility’s established policies and procedures. • Assure that a stock of medications, medical supplies, equipment, etc., is maintained at all times.
