What You'll Do

• Provide skilled, one-on-one nursing care in the comfort of your patient's home
• Administer and manage medications
• Monitor and document patient conditions
• Perform treatments as ordered by the physician
• Educate and support family members
• Collaborate closely with the Phoenix clinical team for ongoing guidance and support

What You'll Need

• Active Illinois RN or LPN license
• We'll help you obtain your Missouri nursing license by covering the cost and guiding you through the process
• Strong assessment and communication skills
• A compassionate, dependable, and self-motivated heart for serving others
• Valid driver's license and reliable transportation

Preferred Qualifications Role Purpose The Staff LPN Ambulatory is responsible for providing direct patient care under the direction of a physician or other licensed provider in an ambulatory care setting.
