Evening Shift Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Tucker House Nursing and Rehabilitation, a recognized leader in long-term and post-acute rehabilitation in Philadelphia, Pennsylvania. As a key member of our interdisciplinary care team, you will provide high-quality, individualized care to our residents, while upholding our commitment to excellence in patient care and our philosophy of treating each individual with dignity, respect, and personalized attention.

Responsibilities:

Provide direct patient care to residents in our post-acute rehabilitation setting, including wound care, medication management, and vital sign monitoring
Develop and implement individualized care plans in collaboration with our interdisciplinary care team
Conduct initial and ongoing assessments of residents' physical and emotional needs, reporting findings to the nursing staff and medical team as necessary
Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
Maintain accurate and detailed records of patient care, including vital signs, treatments, and medications
Collaborate with our therapy team to ensure continuity of care and effective communication with residents and their families
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
