Key Responsibilities Conduct Face‑to‑Face encounters for patients entering their 3rd benefit period and beyond. Assess and document terminal prognosis, disease progression, functional decline, and symptom burden. Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions. Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
