About the Role We are seeking a skilled and compassionate Nurse Practitioner to perform required Face‑to‑Face (F2F) encounters for hospice patients approaching benefit period recertification. Key Responsibilities Conduct Face‑to‑Face encounters for patients entering their 3rd benefit period and beyond. Assess and document terminal prognosis, disease progression, functional decline, and symptom burden. Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions. Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices. Maintain timely, compliant documentation aligned with Medicare and state regulatory standards. Communicate findings effectively to physicians and clinical leadership. Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings. Qualifications Current and unrestricted Nurse Practitioner license in New Mexico. National board certification as NP (FNP, AGNP, or similar). Minimum 1-year clinical experience; hospice, palliative care, or geriatric experience strongly preferred. Solid understanding of hospice eligibility criteria and disease trajectories. Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board. 1+ years of clinical nursing experience 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations. Maintain active DEA certification if state allows prescription of controlled substances by APPs 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.
