Registered Nurse (RN) Case Manager – PRN / Part-Time

$38 - $50 / hour

We are seeking an experienced Registered Nurse (RN) Case Manager to join our team. Under the supervision of the General Manager and/or Director of Clinical Services, the RN Case Manager is responsible for conducting client assessments, developing and managing care plans, performing supervisory visits, supporting caregiver training, and ensuring exceptional client service.

Schedule

  • Part-Time, PRN, or Contract Position
  • Flexible scheduling
  • Minimum availability of 8 hours per week
  • Must be responsive to client and agency needs
  • Field-based position requiring travel to client homes


Benefits & Compensation

Assessment Visits

  • Earn $100–$150 per initial client assessment, including in-home evaluation and care plan documentation
  • Higher compensation available for assessments completed within a 24–36-hour response timeframe

Re-Assessments

  • Earn $60–$80 per reassessment visit
  • Reassessments are typically completed every six months and generally require 30–45 minutes

Clinical & Administrative Work

  • Earn $38–$45 per hour for documentation, coaching, caregiver evaluations, CNA skills assessments, and other office-related duties

Private Duty Nursing (PDN)

  • Earn $50 per hour for PDN visits

Additional Compensation

  • Mileage reimbursement for use of personal vehicle
  • Travel time compensation for rural visits
  • Paid training


Key Responsibilities

Client Assessment & Care Planning

  • Conduct intake calls and follow up with prospective clients and families
  • Perform initial in-home assessments and home safety evaluations
  • Develop, implement, and document individualized plans of care
  • Conduct periodic reassessments and update care plans as needed
  • Monitor client conditions and document changes appropriately

Care Coordination

  • Coordinate care between clients, family members, caregivers, physicians, and agency staff
  • Ensure continuity and quality of care services
  • Educate clients and families regarding health, wellness, medications, and fall prevention

Quality Assurance & Compliance

  • Conduct supervisory visits to monitor service quality and caregiver performance
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities
  • Document incidents, complaints, injuries, infections, and other quality-related events
  • Ensure compliance with agency policies and healthcare regulations

Staff Development

  • Conduct CNA skills assessments for new hires
  • Provide training, coaching, and support to caregiving staff
  • Assist with caregiver onboarding and orientation when needed

Documentation

  • Maintain accurate electronic client records in WellSky
  • Complete assessment notes, care plans, medication profiles, and visit documentation in a timely manner.


Minimum Qualifications

  • Active and unrestricted Registered Nurse (RN) license
  • Minimum of three (3) years of experience in:
  • Home Health
  • Hospice
  • Rehabilitation
  • Home Care
  • Private Duty Nursing (PDN)
  • Valid driver's license and reliable transportation
  • Ability to travel to client homes throughout the service area
  • Strong computer skills and ability to document electronically

PRN Registered Nurse (RN) Nurse in Duncan

$38 - $45 / hour
Comfort Keepers is a licensed home care agency that serves seniors and veterans across Oklahoma. Under the supervision of the General Manager and/or Director of Clinical Services, the Registered Nurse or RN Case Manager is responsible for assessing & monitoring patients. RN performs initial patient assessments & home safety evaluations, designs, enters & implements the plans of care, and conducts periodic re-evaluations (every 3-6 mos). The RN interacts with clients, their spouses, family members & significant others, and coordinates with other healthcare providers... all while maintaining standards of professional conduct. RN provides case management AND customer service, and provides advice & information on health & wellness, medications and fall prevention. Specific Nursing Duties: Performs intake calls and helps with follow-up with potential clients ("prospects") Conducts initial assessments / in-home visits, performs re-assessments, home safety evaluations, determines client needs, develops and communicates a plan of care to the family. Conducts regular on-site supervisory visits to evaluate service quality and obtain feedback on Comfort Keeper caregivers. Regularly re-evaluates client plans of care to ensure client needs are met. Documents changes in condition. Informs clients of their rights and responsibilities and respects these rights and responsibilities. Ensures ongoing coordination of care between the client, caregiver, primary care physician, and other Comfort Keepers office staff. Provides skills assessments for new hires (CNAs) and hands-on training for existing caregiving staff. Participates in Quality Assessment Process Improvement (QAPI) activities. Documents incidents, injuries, complaints, infections. May be involved in orientation and onboarding of new caregiving staff along with HR team.
