This is a full-time field position providing home care services across all boroughs of New York as well as Long Island and Westchester.

Cases will be clustered together.

Pay Range: $110,000.00-$125,000.00 annually

Job Details Overview: As a Registered Nurse (Float) with us, you'll play a vital role in planning, organizing, and delivering home care services across multiple locations, including all five boroughs of NYC, Westchester, and Long Island.

Responsibilities:

Conduct comprehensive assessments of patients and families at regular intervals to determine home care needs.
Provide thorough physical assessments and medical histories.
Continuously reassess patient needs and adjust care plans accordingly.
Collaborate with physicians to develop personalized care plans based on nursing diagnoses.
Involve patients and their families in the planning process.
Implement preventive and rehabilitative nursing procedures.
Administer medications and treatments as prescribed.
Offer counseling and health education to patients and families.
Identify discharge planning needs and implement plans prior to discharge.
Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

Qualifications:

Current licensure in the State of New York.
CPR certification and a valid driver's license.
Graduation from an accredited school of nursing.
Preferably, a minimum of one year of recent acute or long-term care experience, ideally in an institutional setting.
Bachelor's degree preferred, with at least one year of experience in home care.
While management experience is not required, you will be responsible for supervising Home Health Aides and Personal Care Aides.
Strong observation, verbal, and written communication skills.
Excellent problem-solving abilities and basic math skills.
Nursing skills per competency checklist.
