Pay Rate: $35–$40 per hour, based on experience

Skilled Nursing and Rehabilitation Center is seeking a skilled, compassionate, and detail-oriented Wound Care RN/LPN to join our dedicated nursing team. This position is ideal for an experienced nurse who is passionate about wound management, skin integrity, and providing exceptional, resident-centered care.
The Wound Care RN/LPN will play an important role in assessing residents, providing wound treatments, monitoring healing progress, preventing skin breakdown, and collaborating with the interdisciplinary team to achieve positive clinical outcomes.
Key Responsibilities
- Assess, treat, and monitor a variety of wounds, including pressure injuries, surgical wounds, burns, ostomies, and trauma-related wounds
- Perform routine skin assessments and identify changes in skin integrity
- Provide wound care treatments according to physician orders, care plans, and facility protocols
- Maintain accurate, timely, and thorough clinical documentation
- Identify residents at risk for pressure injuries and implement appropriate preventive interventions
- Monitor wounds for signs of infection and report changes in condition promptly
- Educate residents, families, and nursing staff regarding wound prevention and treatment
- Collaborate with physicians, nursing staff, dietary, rehabilitation, and other interdisciplinary team members
- Support wound healing by monitoring nutritional and hydration needs
- Assist with pain management related to wound care and treatment
- Monitor and document wound progress and response to treatment
- Promote compliance with infection prevention practices, regulatory requirements, and facility policies
Qualifications
- Active, unencumbered RN or LPN license in the State of Florida
- Minimum of 2 years of nursing experience
- Previous wound care, skilled nursing, long-term care, rehabilitation, or related clinical experience preferred
- Strong knowledge of wound care principles, skin integrity, infection prevention, and clinical documentation
- Excellent clinical assessment, communication, and organizational skills
- Strong attention to detail and ability to prioritize multiple responsibilities
- Compassionate, professional, and resident-focused approach
- Ability to work independently and effectively as part of an interdisciplinary team
