Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

 

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

 

What You’ll Do

 

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments

• Develop and manage individualized, comfort-focused care plans

• Deliver skilled nursing care with a focus on pain and symptom management

 

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team

• Collaborate with physicians, social workers, chaplains, and aides

• Monitor patient condition and adjust care plans to maintain comfort

 

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey

• Help families navigate complex decisions with empathy and clarity

• Build meaningful, trusting relationships in deeply personal moments

 

Drive Quality & Compliance

• Maintain accurate, timely documentation

• Participate in IDT meetings and care planning

• Support quality and compliance standards

 

What You Bring

• Active RN license (or compact eligibility)

• Graduate of an accredited nursing program

• Minimum 1 year of clinical nursing experience

• Strong clinical judgment and compassionate care approach

• Ability to manage a caseload independently

• Excellent communication skills in sensitive situations

• Basic EMR proficiency

• Current CPR certification

• Valid driver’s license and reliable transportation

What You’ll Do Coordinate and oversee hospice patient care across the interdisciplinary team, including contracted providers Serve as the central point of communication between physicians, caregivers, patients, and internal teams Manage physician orders and ensure timely entry, approval, and alignment with the plan of care Partner with Intake to support timely admissions and follow up on pending referrals Continuously assess patient needs and ensure appropriate reassessment with any change in condition Lead development, implementation, and updates of individualized hospice plans of care Support patients and families through education, symptom management, and end-of-life care planning Review clinical reports (occurrence, infection control, OnCall) and take appropriate action Participate in and contribute to IDG meetings, supporting care planning and decision-making Perform in-home visits as needed to support care delivery and team alignment Ensure compliance with hospice regulations, documentation standards, and quality expectations Identify trends or gaps in care and escalate to Branch leadership with recommendations What We’re Looking For Active RN license in state of practice Graduate of an accredited nursing program (BSN preferred) Previous Hospice or Home Health experience required, with strong understanding of end-of-life care 2+ years of leadership, supervisory, or care coordination experience preferred Proven ability to work within an interdisciplinary team environment Strong clinical judgment, particularly in symptom management and patient comfort Excellent communication skills - able to support patients, families, and care teams through complex and emotional situations Valid driver’s license, reliable transportation, and current CPR certification Competitive Pay & Benefits A compensation package that recognizes your expertise and impact. What You'll Do As a Hospice Travel RN, you'll deploy to VitalCaring hospice branches based on staffing and operational needs, including periods of growth, vacancies, acquisitions, onboarding, and critical staffing needs. You'll: Provide skilled nursing care to Hospice patients. Complete comprehensive assessments and develop individualized plans of care. Manage patient caseloads and coordinate care with physicians and interdisciplinary teams. Provide education and support to patients, families, and caregivers. Support recovery and independence for Home Health patients and comfort-focused, end-of-life care for Hospice patients. Quickly integrate into new teams, workflows, and patient populations. Help stabilize branch operations and promote consistent clinical practices and patient experiences across markets. Complete timely, accurate clinical documentation and maintain regulatory and quality standards. Participate in on-call responsibilities as assigned. What We're Looking For This role is ideal for an experienced clinician who is adaptable, confident, independent, and ready to go where they're needed most. Required Qualifications: Graduate of an accredited school of nursing. Current, unrestricted Registered Nurse (RN) license. Active Compact RN License (NLC), or willingness and ability to obtain Compact Licensure within the established timeframe. Current BLS/CPR certification. Minimum of 3 years of progressive Hospice nursing experience . Valid driver's license and reliable transportation. Ability and willingness to travel extensively based on business need - anticipated 75+% travel , depending on assignment. Previous travel nursing experience preferred. Experience supporting multiple branches, markets, acquisitions, or startup environments preferred. BSN and/or CHPN certification preferred. What to Expect Assignments will vary based on organizational need and may include local, regional, and interstate travel . Overnight travel will frequently be required, and some assignments may include weekends or on-call support.
