Job Responsibilities

Delivering quality nursing care across post-acute facilities
Adhering to established resident care plans
Reviewing and staying current on resident charts and medical records
Tracking residents' physical and mental wellbeing and routinely monitoring vital signs
Conducting intakes and carefully documenting residents' medical histories and health concerns
Properly verifying and administering medications while monitoring their impact on residents
Taking part in IV setup, immunization administration, and phlebotomy procedures
Supporting the maintenance of a safe and welcoming facility environment

Qualifications and Skills

Current and applicable state-issued LPN license
Completion of an accredited LPN program
A minimum of 6 months of experience in an LPN or related role
Ability to lift a minimum of 50 lbs
