Summary

A clinically competent, registered, professional nurse who coordinates and directs the home health patient's care based on individual patient needs through the nursing process of assessment, planning, intervention, implementation, and evaluation, and under the direction of a physician or APP. Educates patients, families, caregivers and community providers to safety perform care. Provides follow up by evaluating effectiveness of the home care plan and monitoring patient/family response to the plan. Effectively interacts with patients, significant others, and all other health care team members, while maintaining standards of professional nursing. (Washington State Nurse Practice Act, WAC, ANA Code of Ethics).

Essential Functions

  1. Perform related assignments as requested in order to promote positive visitor, family, and co-worker relations.

  2. Coordinates and provides high quality patient care in a caring and compassionate manor, utilizing the nursing process and unit specific standards of practice.

  3. Accurately collects patient information needed to provide safe, high quality care. Completes holistic ongoing assessments of patient’s status and responds appropriately.

  4. Develops, implements and continually reassesses patient’s individualized plan of care.

  5. Provides nursing care for patients including administration of medications, completion of nursing procedures and/or treatments.

  6. Determines and completes appropriate nursing actions to meet care goals; including the physical, psychological, social and rehabilitative needs of the patient.

  7. Teaches patient/family and/or significant others appropriate health information in a timely manner and shares written informational material.

  8. Continual evaluation of discharge plan for appropriateness, educational needs, and changes as needed.

  9. Documents care provided, assessments, plan of care, interventions and responses accurately in the patient’s medical record.

  10. Demonstrates ability to handle emergency situations in a prompt, precise and professional manner.

  11. Actively maintains current knowledge base of workflows, process and evidence-based procedures.

  12. Delegates appropriately and oversees care provided by non-licensed assistive personnel.

  13. Participates in performance evaluation/improvement processes with Director/Clinical Manager. Assumes responsibility for own professional development and personal growth.

  14. Consistently functions within the guidelines of the Washington Nurse Practice Act, unit specific standards of practice, and keeps current with the legal aspects of nursing.

  15. Communicates and collaborates with members of the healthcare team including physicians, patients and families to facilitate effective patient care and treatment.

  16. Maintains a clean and safe environment for patient and staff according to hospital and unit policies.

  17. Actively participates in quality improvement programs.

  18. Adheres to Confluence Health policies and unit standards as applicable.

  19. Required to maintain active Basic Life Support (CPR) certification.

  20. Performs other duties as assigned.

Qualifications

Required:

  • Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499).
  • Basic Life Support (CPR) certification (within 90 days of hire).
  • Current, valid driver's license and access to an insured vehicle with the minimum coverage by law.
  • Must possess basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.

Desired:

  • Bachelor's Degree (BSN).

Summary The Infusion Therapy RN is responsible for the comprehensive holistic care of oncology and non-oncology patients being infused with chemotherapy, biologic therapy agents, blood products, fluids, and other medications. The nurse provides care coordination and direct patient care; consults with physicians and colleagues; and educates patient/family on symptom management and new therapies, for oncology and non-oncology patients, treated in the infusion department. This position provides longitudinal comprehensive nursing care to oncology/hematology patients and to non-oncology patients requiring infusion therapy and complex cancer treatments. This position works in collaboration with all members of the healthcare team to achieve the best possible outcomes for the patient. Position Reports To : Clinical Practice Manager Essential Functions Demonstrates a high level of understanding of oncology illnesses and treatment modalities. Demonstrates the ability to assess baseline physical, emotional and social needs. Provides clinical and technical oversight to patients requiring infused or injected chemotherapy, biologic therapy, blood products, and other agents: Performs a nursing assessment prior to and during each infusion or treatment per policy and procedure of the medical center. Has knowledge of agents infused including the potential drug toxicities, side effects, proper methods of administration, and interventions necessary for the safety of the patient. Safely prepares infusion and pump set-up for each drug that is infused. Follows parental administration policy and procedure of the medical center for the medications given. Monitors patient during and after the infusion, observing for any adverse effects. Provides anaphylactic/life-saving interventions to potential hypersensitivity reactions following correctly administered medications. Provides comprehensive care to patients as part of the overall care team. Manages patient symptoms, concerns and/or reactions and effectively communicates the entire picture to nurse navigator and/or provider for that patient. Demonstrates ability to develop a plan of care to respond to patient symptoms and reactions resulting from the disease or the treatment. Demonstrates capability to multitask with accuracy and deal effectively with unpredictable situations. Demonstrates ability to effectively intervene and direct colleagues during a patient crisis. Provides patient and family education about treatments and side effects, procedures such as bone marrow aspiration and ommaya taps/intrathecal chemotherapy in collaboration with the other members of the healthcare team. Informs Clinical Practice Manager of high-risk patient situations. Reviews and implements the treatment and therapy plans. Maintains a working knowledge of current protocols related to the patient population. Knows research resources and how to access. Takes an active role in professional development to stay current with medications, oncology care and treatment and current standards of ONS, ASCO, and INS. Serves as resource in their area of expertise for staff and colleagues. Actively engages in PI and LEAN processes. Demonstrates effective and professional communication and collaboration with colleagues and providers. Assures the safety of patients and other staff. Follows all policy and procedures of the Medical Center related to verification of orders and products when administering chemotherapy, biotherapy, and other infusion agents. Demonstrates an awareness of the potentially hazardous nature of many of the agents administered and follows safe handling and disposal policy and procedures of the medical center. Prepares and protects patients and families during infusions through the strict adherence of the infection control policy, universal precautions, and standards applied to procedures of the Medical Center and the Oncology Department.
