QUALIFICATIONS & ATTRIBUTES
Current Licensed Practical Nurse (LPN) license in the state of employment.
Minimum of one year of LPN experience in an acute care setting is required; two years preferred.
Home health experience is preferred.
Ability to effectively communicate and create positive impressions with patients, families, physicians, and coworkers.
Ability to remain calm, patient, and accommodating in a variety of patient care situations.
Compassionate and caring approach when working with patients and families.
Knowledge of nursing best practices.
Ability to make appropriate nursing judgments within the scope of practice.
Ability to assess situations and identify appropriate solutions.
Detail-oriented and observant.
Strong work ethic with the ability to prioritize responsibilities and manage time effectively.
Ability to follow directions and work collaboratively as part of a multidisciplinary team.
Valid driver's license and automobile insurance with the candidate listed as a driver.
MAJOR AREAS OF RESPONSIBILITY
Provides skilled nursing care to patients in their homes as ordered by the attending physician.
Provides care according to established orders, care plans, and goals developed by the RN Case Manager.
Observes, assesses, and monitors patient conditions and reports changes to the RN Case Manager.
Administers medications and treatments as prescribed and within the LPN's scope of practice.
Educates and counsels patients and families regarding disease processes, medications, treatments, and strategies for managing their condition at home.
Incorporates patients and families into the development and implementation of the plan of care.
Helps reduce avoidable hospitalizations by identifying changes in patient condition, providing appropriate education, and reinforcing preventative measures.
Develops positive relationships with patients and families while providing compassionate, patient-centered care.
Communicates effectively with the RN Case Manager, physicians, schedulers, and other members of the care team.
Completes clinical documentation in accordance with agency policies, Medicare requirements, and applicable federal and state regulations.
Documents all aspects of patient care, assessments, treatments, interventions, and patient education accurately and timely.
Maintains an active LPN license in the state of employment.
Communicates schedule or patient-related changes with the appropriate team members.
ESSENTIAL FUNCTIONS
Travels to patients' homes and other assigned locations to provide scheduled visits.
Maintains reliable attendance at assigned patient visits.
Completes documentation for patient visits within required timeframes and in accordance with Medicare guidelines.
Completes and submits all required clinical documentation accurately and within established company requirements.
Provides skilled nursing services within the LPN's scope of practice and in accordance with applicable federal and state regulations.
Manages assigned visits within the designated geographic area based on patient needs and agency requirements.
Participates in required on-call responsibilities and responds appropriately to patient needs.
