Responsibilities

1. Supports patient and families through procedures, medication therapy, emotional and financial issues, etc.2. Provides telephone triage, prescription needs, and notification and implication of lab results.3. Arranges for lab work and procedures. Provides timely notification to families of the results of test procedures, radiology studies, blood work, and any other tests ordered by the team.4. Assists with patient and family needs and patient education materials.5. Documents patient problems and progress.6. Participates in interviewing and evaluation of staff.7. Works with Nurse Manager to ensure adequate nursing coverage.8. Acts as multi-disciplinary liaison with staff from radiology, laboratory, financial services, pharmacies, medical insurance companies, referring providers, and the in-patient units.9. Assists with training and education of nursing students, medical students, and medical residents.10. Participates in relevant educational offerings to enhance effectiveness and achieve required in-service training, and certification.11. Performs other duties as requested or assigned.

Qualifications

  • Graduate from an accredited Nursing Program required.
  • Bachelor of Science Degree in Nursing (BSN) with 3 years of clinical experience required.
  • Masters of Science Degree in Nursing (MSN) strongly preferred.

Required Licensure/Certifications

  • Licensed Registered Nurse with New Hampshire eligibility.
  • Basic Life Support (BLS) Certificate required
  • Certified Wound Care Nurse (CWCN) within one year of hire date.

Overview Integrate emergency care for individual patients and implement nursing standards in the processes of care. Nurse Manager -Family Medicine Newport Health Center

Overview At the direction of the Director of Clinical Services, and in collaboration with Ambulatory Leadership, provides guidance and leadership for the clinical practice of the staff in the ambulatory settings at Newport Health Center (NHC). Accountable for the quality of nursing care service, in accordance with the NH Nurse Practice Act, hospital policies, and the NH Board of Nursing rules for delegated nursing tasks. Collaborates with Medical Director, providers and Practice Leadership to assure development and ongoing competence of the clinical support staff. Works closely with the NLH Primary Care Operations team to build consistent clinical processes across locations. Collaborates with the CNO to assure that system nursing practice goals and expectations are met. Responsibilities Clinical Operations Directly responsible for operational and performance management of RN/LPN, CMA, and other clinical staff in the NLH Medical Group Practice (direct reports). Manages Clinical Schedule, Time Away, and Payroll for Direct Reports, and collaborates with Practice Leadership to provide consistent appropriate clinical coverage to NLMG Providers. Develops and implements program for the onboarding of new clinical staff. Provides leadership in emergent situations and in those that require activation of the Emergency Activation Plan. Maintains up-to-date working knowledge of emergency preparedness in order to function as a resource to staff. Maintains clinical skills in order to provide direct care as needed. Provides clinical consultation and assistance upon request to other patient care areas at NLH and NHC Develops, in collaboration with the Practice Leadership, CNO and nurse leaders, programs and strategies to assure ongoing competency of RN clinical staff. Maintains professional competence through participation in education programs and workshops. Collaborates with Clinical Educator in all clinical education opportunities and mandates. Quality Accountable for working with staff to assure Rural Health Center (RHC) survey readiness. Charge Nurse - Emergency Department

Overview The ED Lead/Triage Nurse is a vital leadership and clinical role responsible for overseeing shift operations in the Emergency Department, ensuring efficient patient flow, safe care delivery, and effective team coordination. This role also involves assessing and prioritizing patients upon arrival through systematic triage, initiating appropriate care pathways, and facilitating communication between healthcare teams. Responsibilities Lead Nurse Responsibilities: Leadership & Oversight Serve as the clinical leader and primary resource for nursing staff during assigned shifts. Coordinate staffing assignments based on patient acuity and unit needs. Manage shift operations, including delegation of tasks and monitoring workflows. Support staff in escalating clinical concerns to providers or leadership as needed. Patient Flow & Throughput Oversee the movement of patients through the ED, from arrival to disposition (admission, transfer, or discharge). Collaborate with bed management, inpatient units, and ancillary departments to optimize patient flow. Address and mitigate bottlenecks or delays in real-time. Resource Management Ensure appropriate availability of supplies, equipment, and staffing resources. Initiate and lead code responses, disaster preparedness actions, and emergency protocols. Communication & Team Support Act as a liaison between ED staff, physicians, other departments, and hospital leadership. Facilitate shift handoff and ensure continuity of care through effective communication. Provide coaching, mentorship, and conflict resolution as needed. Provide coverage for breaks to ensure downtime for our staff. Triage Nurse Responsibilities: Initial Patient Assessment Perform comprehensive triage assessments using standardized triage protocols (e.g., ESI—Emergency Severity Index). Determine the priority level for care based on patient acuity and resource needs. Clinical Decision-Making Identify high-risk patients and initiate life-saving interventions per standing orders or protocols. Communicate critical findings promptly to the care team. Documentation & Data Entry Accurately document triage assessments, vital signs, chief complaints, and other pertinent clinical information in the EHR. Patient Experience & Safety Provide clear instructions and explanations to patients and families regarding wait times, processes, and care expectations. Ensure infection control and safety measures are adhered to in the waiting area and triage spaces. Qualifications Education: Graduate of an accredited Nursing Program (ADN or BSN preferred). Licensure: Current Registered Nurse (RN) license. Certifications: BLS and ACLS upon hire. PALS and AVADE within 6 months of hire. TNCC and NRP within 1 year of hire. ENPC is supported by leadership and optional. Experience: Minimum of 2–3 years Emergency Department nursing experience. Prior leadership or triage experience preferred. Skills: Strong clinical assessment and critical thinking skills. Excellent communication, leadership, and team coordination abilities. Proficiency in electronic medical records (EMR/EHR). Ability to manage multiple priorities in a fast-paced environment.
