Position Overview

Heritage Grove Estates is seeking an experienced and dependable Charge Nurse / Nurse Manager to provide clinical leadership, staff supervision, and direct nursing support within our skilled nursing environment.

This position serves as a key clinical leader on the nursing floor and is responsible for ensuring residents receive safe, timely, and high-quality care while maintaining accountability among nursing staff. The Charge Nurse / Nurse Manager will provide hands-on nursing care when necessary, supervise CNAs and other nursing personnel, conduct resident rounds, coordinate changes in condition, and ensure clinical documentation and care delivery meet facility and regulatory expectations.

This is a working clinical leadership position. The successful candidate must be comfortable balancing leadership responsibilities with direct resident care and responding to the changing needs of the nursing department.

Key Responsibilities

Clinical Leadership & Resident Care

  • Oversee daily clinical operations and resident care delivery within the assigned area

  • Conduct routine resident rounds to evaluate clinical status, quality of care, and staff performance

  • Assess residents for changes in physical, cognitive, behavioral, or psychosocial condition and ensure appropriate interventions are initiated

  • Provide direct nursing care, including medication administration, treatments, assessments, and other nursing services within scope of practice

  • Ensure physician orders, care plans, treatment plans, and clinical interventions are implemented accurately and timely

  • Respond appropriately to acute changes in condition and emergency situations

  • Coordinate admissions, readmissions, transfers, and discharges as assigned

  • Promote resident dignity, safety, preferences, and person-centered care throughout the unit

Staff Leadership & Accountability

  • Provide leadership, direction, supervision, and support to CNAs and nursing staff

  • Make appropriate assignments and delegate responsibilities based on resident needs, staff competency, and scope of practice

  • Monitor staff performance and ensure assigned resident care is completed appropriately

  • Provide real-time coaching, education, and corrective guidance when performance concerns are identified

  • Communicate employee performance or disciplinary concerns to nursing leadership and Human Resources as appropriate

  • Assist with orientation, training, and development of newly hired nursing team members

  • Promote professional communication, teamwork, accountability, and respectful workplace expectations

  • Assist with staffing coordination, schedule adjustments, and call-in coverage when necessary

Documentation & Care Coordination

  • Ensure nursing documentation is accurate, complete, timely, and reflective of residents' current conditions and services provided

  • Review electronic medical record documentation for completeness and follow up on identified concerns

  • Communicate changes in condition and significant clinical concerns to physicians, responsible parties, and appropriate interdisciplinary team members

  • Participate in resident care planning and interdisciplinary meetings as assigned

  • Collaborate with physicians, pharmacy, therapy, dietary, social services, MDS, and other disciplines to support continuity of care

  • Ensure new orders, laboratory results, diagnostic findings, and clinical recommendations receive appropriate follow-up

Quality, Compliance & Resident Safety

  • Maintain compliance with applicable CMS requirements, Kansas regulations, facility policies, and professional nursing standards

  • Support continuous survey readiness through routine monitoring of clinical practices and documentation

  • Participate in Quality Assurance and Performance Improvement (QAPI) activities

  • Assist with monitoring and reducing falls, infections, pressure injuries, medication errors, avoidable hospitalizations, and other adverse clinical outcomes

  • Support infection prevention practices, including hand hygiene, PPE utilization, isolation precautions, and appropriate infection-control procedures

  • Participate in investigations, audits, corrective action plans, and performance improvement initiatives as assigned

  • Immediately report resident safety concerns, allegations, incidents, accidents, or suspected abuse, neglect, or exploitation in accordance with facility policy and applicable reporting requirements

Qualifications

  • Graduate of an accredited nursing program

  • Current, active RN or LPN license in good standing in the State of Kansas

  • Current CPR/BLS certification

  • Previous skilled nursing, long-term care, rehabilitation, or post-acute nursing experience preferred

  • Previous charge nurse, supervisory, or nursing leadership experience strongly preferred

  • Strong clinical assessment, critical-thinking, and decision-making abilities

  • Demonstrated ability to supervise, delegate, coach, and support nursing personnel

  • Strong understanding of clinical documentation, resident care planning, infection prevention, and regulatory compliance

  • Excellent communication and interpersonal skills

  • Ability to effectively communicate with residents, families, physicians, employees, and interdisciplinary team members

  • Ability to prioritize multiple clinical and operational responsibilities in a fast-paced environment

POSITION DESCRIPTION: Responsible for supporting and carrying out the administration and management of Nursing Services to residents in accordance with orders of the physicians and total needs of the residents.
