UConn Community Health Network Medical Group is seeking a compassionate, clinically skilled, and patient-focused Medical Coordinator – LPN to support patients and clinical teams through telephone triage, care coordination, and clinical support services.
The Medical Coordinator – LPN serves as an important point of contact for patients, assessing their needs, providing appropriate clinical guidance, and determining the appropriate level and timing of care. This position requires strong clinical judgment, critical thinking, excellent communication skills, and the ability to provide reassurance and exceptional customer service to patients and families. This position is full-time, 40 hours per week, Monday-Friday, 8:30 am - 5:00 pm with some flexibility required.

Key Responsibilities
- Triage incoming patient calls, assess clinical needs, and determine appropriate care options.
- Provide patients with clinical guidance, education, and recommendations regarding immediate care, preventative behaviors, and follow-up.
- Determine when patients require urgent, same-day, or next-day appointments based on clinical assessment.
- Provide compassionate, professional, and reassuring communication to patients and families.
- Administer injections and perform other appropriate nursing interventions.
- Implement physician orders and assist with coordinating patient care.
- Notify patients of test and diagnostic results as directed by providers.
- Process prescription refill requests in accordance with established protocols and provider direction.
- Document patient calls, assessments, interventions, and follow-up accurately in the electronic medical record.
- Collaborate with physicians, nurses, medical assistants, and other members of the healthcare team.
- Maintain patient confidentiality and comply with applicable policies, procedures, and regulatory requirements.
- Provide exceptional customer service while managing multiple priorities in a fast-paced clinical environment.
- High School Diploma or equivalent.
- Graduate of an accredited School of Nursing.
- Current Connecticut State Practical Nursing License (LPN) required.
- Prior telephone triage experience required.
- Strong clinical judgment, critical thinking, and patient assessment skills.
- Excellent verbal and interpersonal communication skills.
- Superb customer service skills with the ability to demonstrate empathy, patience, and professionalism.
- Ability to reassure and calm concerned patients and callers.
- Strong organizational skills and ability to effectively prioritize multiple patient needs.
- Proficiency with electronic medical records highly desired.
