Duties & Responsibilities

Purpose of Position:

Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and provision of care supportive to and restorative of life and well-being.

Examples Of Typical Tasks

  • Identifies and discriminates between physical and psycho-social signs and symptoms. Selects and performs those therapeutic measures which are essential for effective execution of the nursing regimen for assigned patients/clients.
  • Develops, implements and continuously evaluates the nursing regimen for assigned patients/clients.
  • Records and maintains nursing care plans and progress notes on patients/clients to ensure continuity of care.
  • Interprets and reports responses of patients/clients to appropriate members of the health team.
  • Participates with interdisciplinary health team members to plan a comprehensive patient care program.
  • Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel.
  • Provides teaching, guidance and direction to allied nursing personnel in rendering patient care.
  • Participates in defining, implementing, maintaining and interpreting standards of nursing practice.
  • Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice.

Minimum Qualifications

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team: Collaborates with the appropriate Physician or Provider on patient’s plan of care; Accurately interprets and implements treatment regimen as prescribed by the Physician or Provider; Assists the Physician or Provider during treatments and/or diagnostic procedures; Keeps the Patient Care Manager/designee and/or Physician or Provider abreast of changes in patient’s condition and/or treatments, as appropriate; Uses clinical judgment in delegating assignments in providing patient care, and ensures that assignments are completed in a timely fashion. Provides wound care and care for pressure ulcers (Stages I–IV): Performs preventive skin care measures; Applies simple dressings, maintaining principles of aseptic technique; Applies warm & cold compresses, consistent with facility procedure; Performs irrigations, consistent with Facility procedure; Performs other procedures related to skin care, as necessary. Administers medications correctly and safely: Correctly identifies medication (action; dosage; side effects and implications for the patient); Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V. drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. Participates in patient and family education: Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-procedural or operative care; coordinates appropriate referrals for follow-up care, specialty services, or community resources; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current license to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
