Weekend Integrated Support Nurse (Baylor) – RN/LPN

Location: Brockton/Easton Area (Travel Required)
Schedule: Saturday & Sunday, 7:00am–11:00pm
Hours: Baylor Schedule (32 hours worked; paid equivalent of 40 hours in accordance with BAMSI Baylor Program requirements)
Pay Rate: $42/ Hour

What You’ll Do:

The Weekend Integrated Support Nurse provides direct clinical support, healthcare oversight, quality assurance, and operational assistance across BAMSI residential programs during weekend hours.

Weekend Clinical Support & Response

• Serve as a visible nursing presence across assigned residential programs throughout the weekend
• Respond to clinical concerns and provide nursing assessments and interventions within scope of practice
• Provide consultation and guidance to residential staff regarding illness, injury, medication concerns, and changes in condition
• Triage healthcare concerns and coordinate with healthcare providers, pharmacies, hospitals, and leadership as needed
• Respond to urgent clinical calls and support staff with time-sensitive healthcare issues
• Escalate significant clinical concerns to the Director of Integrated Health Services and operational leadership
• Provide support during unexpected nursing needs or staffing shortages
• Travel between residential programs based on clinical priorities and organizational needs

Admissions & Transitions of Care

• Support weekend admissions, hospital discharges, emergency department returns, and other transitions of care
• Review physician orders, medications, treatment plans, and discharge documentation
• Coordinate with pharmacies, healthcare providers, nursing leadership, and residential teams to ensure continuity of care
• Identify and address medication discrepancies, missing orders, or other concerns affecting safe transitions

Program Rounds & Quality Monitoring

• Conduct clinical rounds and program spot checks throughout residential locations
• Assess healthcare practices, medication systems, infection control measures, and compliance with clinical protocols
• Observe implementation of healthcare plans and physician orders
• Provide immediate coaching and corrective guidance when concerns are identified
• Communicate trends and significant findings to nursing and operational leadership

Medication Administration & MAP Compliance

• Support compliance with Massachusetts Medication Administration Program (MAP) requirements
• Conduct medication audits and medication administration observations
• Review Medication Administration Records (MARs), physician orders, controlled medication documentation, and related records
• Assist staff with medication-related clinical problem-solving
• Review medication occurrences and ensure appropriate reporting and follow-up
• Provide coaching and education to MAP-certified staff
• Escalate significant medication errors or safety concerns immediately

Documentation & Chart Audits

• Conduct healthcare record reviews for completeness, accuracy, and compliance
• Review nursing notes, treatment records, laboratory results, healthcare appointments, and follow-up documentation
• Identify documentation gaps and support staff with corrections and improvements
• Ensure significant weekend clinical activity is documented and communicated appropriately

Residential Operations Support

• Partner with residential leadership to resolve healthcare-related operational challenges
• Serve as a clinical resource for Program Managers, Directors, on-call leadership, and direct support staff
• Assist with clinical decisions that impact staffing, transportation, programming, and support needs
• Support continuity of care during emergent operational situations

Communication & Collaboration

• Communicate with individuals served, families, guardians, healthcare providers, and interdisciplinary team members as appropriate
• Maintain ongoing communication with nursing and operational leadership throughout the weekend
• Provide detailed weekend handoff reports to weekday nursing and operational teams
• Communicate significant incidents, hospitalizations, medication events, and safety concerns through established protocols

Quality Assurance & Risk Management

• Promote compliance with regulatory, licensing, accreditation, and BAMSI healthcare standards
• Identify medication, documentation, infection control, safety, and clinical risks
• Support corrective actions and continuous quality improvement efforts
• Maintain infection prevention standards, safety practices, and universal precautions

Staff Support & Education

• Serve as a mentor and clinical resource for nurses and residential staff
• Provide real-time coaching related to medication administration, healthcare protocols, chronic conditions, documentation, and infection control
• Support orientation and development of newer nursing staff
• Help ensure consistent clinical practices across multiple programs

What You Bring:

Licensure

• Current Massachusetts Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing required
• Current CPR certification required

Experience

• Two to three years of nursing experience preferred
• Experience in residential, community-based, developmental disability, behavioral health, or human service settings strongly preferred
• Experience supporting individuals with complex medical, developmental, or behavioral health needs preferred
• Experience with medication administration systems and MAP requirements preferred

Skills & Qualifications

• Strong clinical assessment and nursing judgment skills
• Ability to respond effectively in urgent and changing situations
• Strong medication management and clinical documentation abilities
• Knowledge of MAP regulations, DPH requirements, and community healthcare practices
• Ability to conduct audits, identify compliance concerns, and implement corrective actions
• Excellent communication and interpersonal skills
• Strong organizational and follow-through abilities
• Ability to balance clinical intervention, quality assurance, and operational support responsibilities
• Comfortable working independently across multiple program locations

Additional Requirements

• Valid driver's license required
• Reliable transportation and ability to travel between program sites throughout the shift
• Ability to meet all credentialing, background check, and regulatory requirements
• Ability to work the Baylor weekend schedule consistently

Physical Requirements

• Ability to lift, push, pull, and maneuver up to 25 pounds
• Frequent standing, walking, bending, and stair climbing
• Ability to travel between multiple residential locations throughout a shift
• Visual and auditory acuity sufficient to perform clinical assessments and nursing responsibilities
• Ability to respond directly to situations requiring immediate clinical intervention
