Overview

Collects and reports clinical data to CMS and national registries ensuring accuracy and reliability of submissions. Serves as a resource on clinical data related to registry participation and CMS measures . Provides feedback and interpretation of registry reports and benchmarks to staff and physicians. Queries databases, analyzes data and reports information to appropriate stakeholders. Participates in multidisciplinary improvement initiatives by providing information to support the rationale for practice changes.

Responsibilities

Accurately abstracts and cleans data as needed
Provides feedback and communication to physicians, management and staff on measures and comparative data
Participates in committees and/or multidisciplinary teams to analyze contributing factors, identify opportunities for improvement in documentation or changes in practice patterns
Assures abstraction and submissions are done in a timely fashion
Stays current and acts as a resource on definitions used for abstraction
Assembles routine reports and distributes to appropriate stakeholders, committee and management
Provides ad hoc reports as requested out of registries & local databases as requested

Qualifications

Education:

Graduate of an accredited school of nursing or IA Board of Nursing approved nursing program- LPN or RN

License(s)/Certification(s):

Current licensure in good standing to practice as a Registered Nurse or Licensed Practical Nurse in Iowa

Experience:

3 years in healthcare

Responsibilities Patient Assessment & Triage Evaluate incoming patient phone calls and electronic messages to determine the urgency of symptoms such as abdominal pain, rectal bleeding, changes in bowel patterns, wound concerns, or post-operative complications. Provide telephone triage and guide patients to the appropriate level of care, including clinic visits, emergency evaluation, or home care instructions. Assess preoperative readiness and identify risk factors that may impact surgical outcomes. Care Coordination Assist in coordinating preoperative testing, surgical scheduling, and postoperative follow-up care. Collaborate with colorectal surgeons regarding urgent issues such as infection, bowel obstruction symptoms, ostomy concerns, or wound complications. Coordinate care with primary care providers, gastroenterologists, wound/ostomy nurses, oncology, and other specialties as needed. Patient & Family Education Educate patients and caregivers on colorectal conditions such as colorectal cancer, inflammatory bowel disease, diverticulitis, hemorrhoids, and anorectal disorders. Provide detailed preoperative and postoperative teaching, including bowel prep instructions, pain management, incision care, ostomy care, diet modifications, and activity restrictions. Offer emotional support and resources to patients adjusting to new diagnoses or life changes such as ostomy placement. Clinical Documentation & Follow-Up Accurately document all patient interactions, assessments, surgical education, and care plans in the electronic health record (EHR).
