As an RN Case Manager at Novant Health Mint Hill Medical Center, you will serve as a leader of the complex discharge planning process and a valued member of the healthcare team. You will support the needs of the team during the shift you are working, allowing you to build relationships throughout the entire hospital. While you can pick up hours any day of the week, weekday availability is especially needed at this time.
Mint Hill Medical Center is a 46 bed facility recently voted in the top 10 community hospitals in the nation. As a PRN member of the case management team, you will be eligible to and encouraged to float to other hospitals in the region as well after orientation. You must be able to orient on a Monday - Friday basis for 4-8 weeks.

What You'll Do
- Complex discharge planning
- Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
- Collaborate with MSW case managers for guidance with pscyhosocially complex cases
- Facilitate coordination of care
- Accept responsibility, authority, and accountability for management of care
- Serve as a resource for discharge planning and care coordination for medically complex cases
- Maintain current knowledge of available community resources/post acute care options
What We're Looking For
- Education: Associates Degree in Nursing required. Bachelors Degree in Nursing preferred.
- Experience: Experience in hospital or rehab case management strongly preferred. Broad acute care bedside experience is a plus.
- Licensure/Certification: Current RN licensure in state of employment, required.
- Additional Skills/Requirements (required): Holiday coverage; ability to train during the week for 4-8 weeks
