JOB HIGHLIGHTS--QMAP

We are looking for outstanding individuals to join the nursing team as a Certified Medication Aide (C.M.A.). You will make a difference in the lives of residents by assisting them with activities of daily living and coordinating their medication administration.

Responsibilities--QMAP

Assistance with Medications for all assigned residents
Assisting with the daily care of the residents, including: Personal care, grooming, hygiene, housekeeping, laundering, social interactions, and meals.
Monitoring the resident’s physical and emotional comfort and responding as needed
Documenting completed tasks such as: Daily shift report, resident records and negotiated service agreement
Confidentially communicating clearly to other staff, such as: Urgent needs for the next shift and all concerns to the Health Care Director

Brentwood by the Bay Assited Living is seeking a part time/ per diem Med Tech/ CMT. The primary role of the Med Tech is to administer medications and provide direct care to residents. The Med Tech Certified reports to the Supervisor in charge and the nursing team leader. PRIMARY FUNCTIONS AND RESPONSIBILITIES · Pass medications according to physician orders and in accordance with facility policy. Administer accurately and timely. · Record medications administered in the resident’s record. · Notify pharmacy (or family when applicable) when medications need to be refilled. · Maintain a clean and organized medication cart and medication room. · Monitor and report any/all side effects of medications to the Supervisor. · Obtain/provide shift report. Record sufficient information related to medication administration to implement accurate changes to medication orders. · Follow comprehensive care plans related to medication administration based on each resident’s needs and his/her comprehensive assessment as assigned. · Participate in in-service education programs as assigned. · Follow facility Residents’ Rights policies and procedures at all times. · Promptly report changes in resident condition to the Director of Wellness. · Observe all facility safety policies and procedures. This individual must perform these tasks as ordered by a physician, under the supervision of a licensed nurse, and in accordance with state and federal rules and regulations. Certified Medication Technician CMT supervises care staff in absence of nurses. Maintains, cleans and organizes the Medication Room. Maintains a liaison with other department supervisors to adequately plan for resident activities. Attends all required training, in-service and staff meetings May be subject to the handling of and exposure to hazardous chemicals. May be subject to exposure to infectious waste, diseases, conditions, etc, including TB and the AIDS and Hepatitis B viruses. Maintain resident confidentiality, staff confidentiality; treat staff and residents with kindness, dignity and respect. Comply with Resident’s Rights rules. Certified Medication Technician CMT - Requirements: A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience. Certified Medication Technician certification required.
