What You'll Do
  • Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans.
  • Perform comprehensive patient assessments and monitor changes in condition.
  • Educate patients and families to promote confidence and independence at home.
  • Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team.
  • Maintain accurate documentation using our AI-supported charting technology.
  • Participate in quality improvement initiatives and ongoing professional development.

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.

What We're Looking For
  • Current Missouri or Compact LPN license.
  • Excellent assessment and communication skills.

  • Valid driver's license and reliable transportation.
  • Home health experience is a plus, but we welcome nurses who are eager to learn.
