Key Responsibilities:
· Administer medications to clients via various methods, including oral and injection, in accordance with service line requirements.
· Perform thorough nursing assessments, including physical and mental health evaluations.
· Accurately document medication administration and nursing assessments, adhering to service line standards.
· Conduct medication administration reviews and ensure house stock of medications is monitored.
· Communicate effectively with clients regarding medication inquiries and manage refill requests.
· Verify client data and medical history, and obtain necessary physical preliminary requirements.
· Complete all required documentation in a timely manner, including safety plans and nursing assessments.
· Act as a resource for nursing and medical issues while overseeing client hygiene and related health concerns.
· Participate in necessary meetings and perform additional duties as assigned by nursing leadership.

Education, Experience, and/or Credential Qualifications:
· Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
· Graduation from an accredited practical nurse program.

Additional Qualifications:
· Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
· Current driver’s license, acceptable driving record, and current auto insurance.

Physical Requirements:
· ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects.

Job Description: The Licensed Practical Nurse (LPN) / Registered Nurse (RN) is responsible for providing nursing services to patients in residential care facilities, behavioral health and/or substance use disorder treatment programs. They work to maintain a safe and therapeutic environment for clients and provide support through various nursing duties across outpatient psychiatry, addiction, and housing services. Conduct MAR reviews and medication counts, monitoring house stock of medications and refill status for housing clients. Prepare pass medications as needed. Ensure that all medical and medication records are up-to-date and accurate, and assist with general lab and phlebotomy duties as needed.
